Submissions must include the following:

Acord applications:

We accept standard Acord applications.
In order to save time on your quote please make certain the commercial application includes prior carrier information, including: limits, premium paid and loss information.
The property application must include the year built with the latest building updates.

Include 3 years current valued Loss Runs with the Acord applications.

Supplemental applications, (see Supplemental Applications):

We require that all applicable Supplemental Applications accompany the Acord applications.

Inspection agreement, (see Forms):

• Include the contact name and phone number of the owner or manager for the location.

• The inspection will include a verification of the sales. The insured must have sales tax filings and financial statements available for the inspector at the time of the inspection. It is important the sales be verified correctly by a knowledgeable person to eliminate the possibility of an incorrect endorsement resulting in a change of premium.

• Axiom Insurance Managers has formed an agreement with Environments of Quality to collect the inspection fees. This will enable the insured to make one check for the insurance and the inspection.
